Dr Tan Jit Seng is the Director of Lotus Eldercare Private Limited.  The company is set up on 1st July 2013, to cater to increasing needs of home eldercare services for the silver population in Singapore. 

Dr Tan graduated in 2002, from National University of Singapore and immediately in his first year of practice encountered SARS outbreak as a House Officer in the public hospital.   He went on for a year of training in managing neurosurgical patients as a medical officer in National Neuroscience Institution, TTSH Campus, both in Neurosurgical Intensive Care Units and general wards.

Thereafter, Dr Tan served out his National Service in Tengah Airbase Medical Center, where he was awarded RSAF Best Safety award and AFMS Best Safety award. Dr Tan resumed his public sector service as a Medical Officer in TTSH Emergency service, TTSH Otorhinolaryngology service, AH Surgical service and finally Yishun Polyclinic. After the stint in polyclinic, Dr Tan joined a 2-doctor private general practice in Marsiling, learning the wide range of skills under a very senior general practitioner. Following the private clinic practice, Dr Tan worked as a full time locum to gain experience of general practice in big corporate groups.

In 2009, Dr Tan joined  Ren Ci Home Care as a home care doctor. In Ren Ci, Dr Tan picked up the skills and medical knowledge of seeing home care patients and running the service. He was also tasked to run Ren Ci Nursing home as well during the one year association with Ren Ci.  However, Ren Ci Home Care programme was terminated after restructuring of Ren Ci.

Dr Tan then set up CODE 4 Private Limited in January 2010 with two other doctors, and continues to see subsidised and private home care patients under one of its branches, CODE 4 Home Care. In that period of 3 and a half years, Dr Tan also supported Kwong Wai Shiu Hospital, seeing her nursing home patients and running her charity clinic for 18 months.

Dr Tan was invited to help out with the new transitional care service in KTPH in March 2012 and supported the programme in its various forms for 5 years, until its restructuring in Feb 2017.  In May 2013, Dr Tan also took up the challenge to support charity home medical services under Lotus Eldercare for Thye Hua Kwan  Ensuite Home Care until May 2015.

CODE 4 Home Care, serving many subsidised patients, was taken over by a charity from July 2013 to boost its operations.  Dr Tan proceeded to incorporate Lotus Eldercare Private Limited to continue to take care of his private long term care patients.  Dr Tan is presently involved in both public and private sector of home eldercare services. In Novemeber of 2015, Dr Tan also started working as a family physician in Mount Alvernia Outreach Medical Clinic part time to see more needy patients and people with disabilities, stationing in Enabling village, a focused venue for disability services under Sg Enable, Singapore until 2020.

Following the Pandemic, in 2021, Dr Tan supported the COVID 19 vaccination efforts by Health Promotion Board, going to various Nursing Homes, Day Care Centers, Disability Home etc to vaccinate the residents. He also helped piloted the Home Vaccination Program under MOH in May to June 2021. In 2022, Dr Tan was asked to support the work in MINDS Clinic based in Blk 2 Queen's Road, helping to review PwID cases and perform Mental Capacity Assessment for deputyship applications. However, from 2023 onwards, due to the increasing workload under Lotus Eldercare Health Services, Dr Tan had to focus on his company fully. 

Dr Tan is an accredited assessor for FDWG, IDAPE, Eldershield, InterRAI and for Mental Capacity Act to do Lasting Power of Attorney or Mental Capacity Assessments.

Teo Shing Yuen (Ms)

Ms Teo graduated from National University of Singapore in 2001.  She started work in Unity as a pre-registration pharmacist.  Upon getting her license, she continued to work with Unity as a retail pharmacist, taking care of the day to day running of the pharmacy on top of her dispensing duties.  During this time, she worked in pharmacies located in different parts of Singapore, serving and advising the heart-landers of Singapore.

She was promoted to Area Manager in 2008, managing and solving issues for the 11 pharmacies under her care, co-ordinating renovations of current stores, opening of new stores and hiring staff.

In 2010, she was re-designated to Operations Manager where she took on new roles of co-ordinating between the different departments for operational issues, setting and writing the SOPs, testing of new IT systems, running BCM for operations and organising social activities for the staff.

Ms Teo left Unity after a fruitful 11 years to join Lotus Eldercare Pte Ltd as Operations Manager.

How We Operate

Our doctor is available from 9.00 am to 5.00 pm. Mondays to Fridays, excluding public holidays, strictly by appointment only.  Please call to arrange for an appointment.

Our charges range from about $250 to $300 per doctor visit per patient for a routine visit for patients under our long term home care service, including procedures and reviews. Medications and expendables will be charged separately if required. All visits are strictly appointment based only. We are not a medical clinic service and will not support services such as immunizations or review urgent hyper-acute conditions. We also only sign CCODs for patients under our long term care.
